What does the situation currently look like? What medical, social supports and limitations currently exist? Perhaps you’ve reached a fork in the road and it is time to consider what needs are currently being met and the areas in which you could use more assistance.
The goal of the assessment process is to provide objective information that will assist you in making informed decisions. When you feel it is time to explore available planning options, phone the Delaware County Office of Services for the Aging (COSA) at 610-490-1300 and ask to speak with someone in the Information & Referral Unit. Over the phone or in person, they will answer general questions on your situation and assist with appropriate service information. Through this initial contact, an assessment may be suggested.
We assess for programs that promote being able to safely remain at home or help you to understand what other options are available. Suggested services may include: home delivered meals, personal care service, home support, shopping assistance, and adult day care.
When a professional COSA assessor makes a home visit they review what benefits you may be eligible to apply for. The information, evaluation and assessment are available at no cost. COSA provides information on resources and programs that will help you decide which way to turn.
